There comes a time in almost everyone’s lives when they begin to need help with their everyday tasks at home. Examples of such tasks might include cooking, cleaning and transportation, or they might include personal care such as bathing, getting around, and help getting dressed.
In some situations, it may just be nice to have another person around for companionship and help. You don’t need a nurse, you just need a little help. You can receive that help with our in-home caregivers.
Every client is a unique individual, and every person has unique needs with respect to types and levels of care. You deserve to have your care needs thoroughly assessed and you deserve to have a personally customized plan of care.
When you are ready to have our professional staff visit you and prepare a complete care needs assessment, we will be happy to come to your home and create a custom care plan just for you.
To best determine the services needed for each client, an thorough in-home care needs assessment will be done by MB Services management staff. During the assessment, details of individual situations will be compiled allowing for the development of a case plan that best suits the clients’ needs. Information that will be gathered should include but is not limited to:
- Medical history and/or medical diagnosis
- Grooming Assistance needs (i.e. toileting, bathing)
- Medications (for reminder purposes)
- Nutritional Information (for dietary preparation)
- Transportation needs (i.e. doctor’s appointments or errands)
- Light Housekeeping needs
- Exercise needs
The MB HomeCare team includes certified nursing assistants (CNAs) certified medicine technicians (CMTs) and compassionate, skillful companions supervised by a registered nurse (RN). They are selected with care to meet the individual’s needs. You can trust that they are carefully screened, credentialed and trained.

Caregivers can help with daily activities, including personal care such as bathing, dressing, toileting, walking and medication reminders, meal preparation, light housekeeping, errands and more. With home care, your loved one gets personalized care and they can carry on their normal routine activities in a very familiar environment-their home. Under physician orders, we can assist with clients’ medication needs with services like these:

Care Transitions (Hospital or Rehab to Home)
Our goal is to work with you to prevent future readmissions back to the hospital!! Coordination with the discharge planner, community resources, transportation home, medication review and assistance with the entire transition back to your home….worry free!
Our RN meets with the discharge planner, receives a copy of your orders, accompanies you home to review orders and medications; sets up a medication plan, develops a customized care plan, reinforces and provides ongoing education and conducts a safety check around the home (including checking for expired food in refrigerator), and provides ongoing oversight of the care delivered by our care team.
This ensures a safe discharge and can be comforting for out of state family to know that MB HomeCare can alleviate that burden. With good management and care, certain chronic diseases such as CHF, Diabetes, COPD and Dementia can be kept in check and avoid ER or hospital trips and reduce hospital readmissions.
We can serve as a companion during your hospital stay and be your voice and ensure your safety. Often times, family can’t be present 24/7 and in this day and time, it’s never more important to have an advocate by your side than when you’re in the hospital or skilled nursing facility. A professional caregiver can be your loved one’s companion, advocate and witness and can also keep a log of everything that happen, for later use if needed.
We can provide short term care coverage periods while your family is away. Often times, a spouse may need surgery or special treatment but there is no one to fill their shoes. MB HomeCare can provide short term coverage so you have peace of mind.
Any client under home hospice services can receive hospice support services from MB HomeCare. The team members from the hospice care organizations only visit for short periods of time during the week. The rest of the time, for a few hours a day up to 24 x 7, all day long, every day, clients under hospice care can receive ongoing home care from MB HomeCare. We work collaboratively with all hospice organizations.

I would also like to thank you for all the wonderful caregivers you provided for her over the past five months. They did so much to help make her stay at the facility happy days. I would especially like you to personally thank Benji, Shardae, Deanna, India, and Rachael for making her last two weeks both in the hospital and under hospice care as comfortable as possible. I would especially like you to thank Rachael for bringing in her violin to play Christmas songs for my wife. One of her last words was “that’s beautiful” after Rachael’s second song. May God bless you and ALL of your caregivers. I would also like to thank you for coming and spending time with me to help ease the pain during my last two hours with my wife. I really miss her.
